2025 Children’s Ministry Permission Form

It is the goal of the Calvary Chapel Children’s Ministry to honor God by providing each child with an opportunity to experience God’s love and understand His Word.

All children who attend Calvary Chapel Mason City are required to have a parent or legal guardian fill out and sign a permission form.

Before services begin, all children who attend must be signed in and out by a parent or legal guardian to ensure the safety of the child and the legal liability of the church.

When participating in Calvary Chapel Mason City activities, I give consent for my child(ren) to be interviewed, photographed, videotaped, or electronically imaged for the purposes of promotional materials, news releases, or other published formats for Calvary Chapel Mason City. The images will be the sole property of the Calvary Chapel Mason City. I hereby release and hold harmless Calvary Chapel Mason City from any claim arising from the use of these images.

I allow Calvary Chapel Mason City Volunteers, when needed, to assist my child in toilet training or change their diapers.

By signing, you give legal consent to Calvary Chapel Mason City Volunteers to care for your child and teach them the Bible in accordance with our belief statements. This includes the Biblical stance on two genders, Biblical sexuality, and marriage between a male and female. 

You also give Calvary Chapel Mason City Volunteers legal consent in the event of an emergency to administer emergency care if needed, to include first aid, CPR, or calling 911. Your signature also acknowledges that you have received, reviewed, and agree with our classroom policy.
